The 1965 legislation Johnson signed created Medicare for older Americans and Medicaid for eligible people with limited means. Congress assembled different financing and eligibility arrangements rather than one universal programme. This is a clear case of expansion by population group: the new protection was substantial, but it left much of the wider insurance system intact. Earlier proposals, particularly Truman's, and the legislative coalition are part of the explanation for its passage.
